General Practice Neighbourhood Fellows

We are inviting expressions of interest from GPs, Nurses, Allied Health Professionals, and Enhanced/Advanced Practitioners working within Derbyshire to apply for one of eight Neighbourhood Development Fellowships. Each fellow will be embedded within one of Derbyshire’s neighbourhoods to support integrated working, amplify patient voice, and lead on local transformation projects.

These fellowships are designed to strengthen neighbourhood-level clinical leadership and problem-solving and address health inequalities through compassionate, inclusive, and values-based approaches.

Background

Neighbourhood working is central to the NHS Long Term Plan and Derbyshire’s place-based strategy. These fellowships aim to build capacity for clinical leadership at the neighbourhood level, enabling better coordination of care, stronger community partnerships, and more responsive, preventative services.

Fellows will work closely with neighbourhood clinical leads and stakeholders across primary care, community services, voluntary sector, and public health. They will also engage directly with patients and communities to ensure that transformation efforts reflect lived experience and local priorities.

Fellowship Overview

The Expectations

The successful Fellows will:

  • Work 1–2 sessions per week over a 9 month period (potentially 12 subject to extension)
  • Collaborate with neighbourhood clinical leadership to support integrated working
  • Bring together stakeholders including patients, carers, and community organisations
  • Deliver a defined project that improves patient care, community delivery or preventative healthcare
  • Identify and address health inequalities within the neighbourhood population
  • Share learning and complete evaluation forms at 3, 6, 9, +/- 12 months
  • Champion compassionate, inclusive, and values-based leadership
  • Champion integrated working and support system-wide improvement

Role Eligibility Criteria

Applicants must:

  • Be able to commit to working 4-8 hours per week/ 1- 2 sessions per week across 9+ months and potentially subject to a 6-month extension
  • A current GP, Nurse, Allied Health Professional, or Enhanced/Advanced Practitioner working within Derbyshire
  • A clinician in a permanent role, locum or bank but working in Derbyshire and actively registered with appropriate professional body
  • Planning to stay in Derbyshire for at least the next 5 years
  • Intending to complete the Fellowship in addition to existing clinical sessions (i.e. a net gain)
  • Not be subject to interim suspension under section 41A of the Medical Act 1983 or equivalent
  • Commit to allocated time and follow the Hub+ QI process including adhering to quarterly project reporting schedule
  • Passionate about inclusive leadership and neighbourhood transformation
  • Able to work independently and communicate effectively
  • Committed to addressing health inequalities and improving community outcomes

Our Offer

  • Hours: 4–8 hours weekly (1–2 sessions)
  • Duration: Fellowship contract up to 31st March 2027. Pending on budget, progress and re-procurement, fellows may be extended past 31st March 2027
  • Location: Hybrid – Base site RTC, RTC Business Park, London Road, Derby, DE24 8UP
  • Contract type: Fellowship agreement as individual contractor paid directly or third-party contractor paid through GP Practice/PCN/Employing organisation
  • Approx rates: Up to £10k per session, total funding £20,000 per fellowship

Fellows will be supported by Hub+ and the Neighbourhood Executive. They will have access to mentorship, learning resources, and opportunities to contribute to national policy and practice development. They will also form a supportive action learning group, to problem-solve, provide mutual support and accountability.
