12:00-13:00 Dr Mary Lewis: Prescribing at End of Life

• Clarify the terms ‘end of life’ and ‘end of life drugs’
• Practicalities around prescribing end of life drugs
• Reflection on the patient and carer experience
  
• Can carers in Derbyshire be trained to give end of life drugs?
  

13:00-14:00 Dr Pauline Love: Proactive Identification in the last 12 months of life

• Define the purpose and scope of Proactive Identification Guidance in recognizing people in their final year or years of life.
• Identify the three main clinical trajectories of expected decline: rapid predictable decline (cancer), erratic decline (organ failure), and gradual decline (frailty and dementia).
• Recognize general and disease-specific clinical indicators that signal a worsening prognosis.
• Understand how to use practical prompts like the "Surprise Question" ("Would you be surprised if this patient were to die in the next 12 months?").
