Community Transformation Fellowships

We are inviting expressions of interest from GPs, Enhanced Practitioners, or Advanced Practitioners working within Derbyshire to support a series of Community Transformation Fellowships. These fellowships will contribute to the aspirations of the NHS Ten Year Plan and Neighbourhood guidance by improving coordination across primary, community, and secondary care.

Background

The NHS Ten Year Plan and neighbourhood working models emphasise the importance of integrated, patient-centred care. These fellowships aim to identify opportunities, solve problems, and support transformation workstreams that improve access to, navigation, coordination and delivery of community care, as well as system efficiency.

Fellowship Overview

Three fellows will be appointed to work collaboratively as part of a transformation team covering workstreams including Local and Central Navigation Hubs, Front Door Turnaround and Intermediate Care Pathways, Urgent Community Response and Virtual Wards.

The Expectations

The successful Fellows will:

  • Work 2 sessions per week over a 9–12 month period
  • Collaborate with Hub+ and Community Transformation leads
  • Lead on assigned transformation workstream supporting colleagues in team and connecting key themes to resolve or escalate
  • Engage with stakeholders across primary, community, and secondary care
  • Identify opportunities and propose solutions to improve care access, navigation, coordination and delivery
  • Attend relevant meetings and contribute to strategic planning.
  • Share learning and complete evaluation forms at 3, 6, 9, +/- 12 months
  • Champion integrated working and support system-wide improvement

Eligibility Criteria

All applicants must be:

  • A current GP, Enhanced Practitioner, or Advanced Practitioner working within Derbyshire.
  • Planning to stay in Derbyshire for at least the next 5 years.
  • Intending to complete the Fellowship in addition to existing clinical sessions (i.e. a net gain).
  • Hold appropriate professional registration and license to practice.
  • Not be subject to any professional suspension or restriction.

The Offer

  • Hours: 8 hours weekly (2 sessions)                            
  • Duration: 9–12 months
  • Location: Hybrid – Base site RTC, RTC Business Park, London Road, Derby
  • Contract type: Fixed Term, starting ASAP
  • Pay: Up to £10k per session, total funding £20,000 per fellowship
